From Square Root
This process is intended to outline a method by which the team can quantify how well we are working together. This process is intended to measure our progress toward meeting the project's minimum Threshold of Success.

Approach
Use the survey taken from The Five Dysfunctions of a Team[1] to quantify how well Square Root is working as a team. The theory is that less dysfunctional teams tend to work together more effectively.
Team Dysfunction Assessment Survey
The survey should be taken by all team members as a part of the Phase Postmortem. The results will be compiled and discussed as a team. The current team leader is responsible for devising strategies for improving the team's effectiveness by decreasing their dysfunctional behaviors. Notes from the discussion and strategies executed by the team leader should be linked or discussed in the reflection section.
Roles
Team leader - administers the survey and compiles the results for discussion. Leads the discussion, creates and executes strategies for improve the team's effectiveness.
Metrics and Data
This process is meant to be a light weight measure. Additional metrics may hamper that goal. Square Root is assuming that the research done by Mr. Lencioni is sound and that the simple survey he created will be good enough for assessing how effectively our team is working together.
Survey Results
This metric is simply the recorded result of the survey. One of two conditions must be met for the team to be considered "good."
- Results should have improved since the last survey
- All areas of the survey score a 6 or better
The lowest individual score for a given category is recorded as the team's score for that category.
